Membership Applications: 

Dojos that would like to become members of Shotokan Karate International - United States Federation (SKIF-USF) need to complete BOTH of the forms below and email them to SKIF-USF.  A representative of SKIF-USF will review the applications and contact you to discuss the details of membership.  Please be aware that membership in SKIF-USF is completely voluntary and can be terminated by either the member dojo or SKIF-USF at any time.  If you have any questions feel free to email SKIF-USF

Form 1: SKI-US Federation Application

Form 2: Affiliated Group/SKIF Tokyo Application

Instructions:  Right-click on each form and select "Save Target As", saving the file to some known location such as your desk-top.  Open the forms from the new location, complete each form with the appropriate information, save it, and email each file to ski_usf@yahoo.com.  Please note that these applications are in editable PDF format and require a PDF viewer (such as Adobe Acrobat) to view and edit.
